Specifications include, but are not limited to: The Department of Health and Social Services, Division of Substance Abuse and Mental Health (DSAMH), is seeking proposals from qualified providers to provide a continuum of housing services to individuals within the state of Delaware meeting the eligibility requirements described under the attached scope of services for each housing type. DSAMH’s goal is to create a coordinated approach to care and enable providers the ability to offer a continuum of services under a single DSAMH contract. This approach is intended to enable individuals to remain with their provider of choice and the flexibility to transition to a different level of care when needs dictate a change in services. The approach is also intended to allow individuals to access coordinated services from providers that may be funded by different state agencies that occupy a role in the life of the consumer.
