Specifications include, but are not limited to: 2.1.The State of Vermont is interested in obtaining bids to meet the following business need(s): Various State agencies currently purchase individual subscriptions for a variety of surveying applications to meet their evaluating needs. These individual subscriptions account to over 750 licenses. This project seeks to centralize one application making it available to all agencies across the State. 2.2.Survey Application requirements include but are not limited to: 2.2.1. Various question response options including but not limited to: multiple choice, textboxes, rating scale, slider scale, radio checkboxes or skip 2.2.2.Conditional based question sorting ex. If answer = no, then go to question xx 2.2.3.Ability to set format in question responses ex. Phone number format 2.2.4.ADA/ §508 compliant 2.2.5.Exportability to word, .pdf, or excel formats 2.2.6.Multiple user functionality including review, comment, testing 2.2.7.Ability to monitor or edit survey after users have begun responding 2.2.8.Response reporting capabilities 2.2.9.Unlimited number of questions permitted with unlimited number of responses to up to 500 surveys responders 2.2.10. Option to only allow one response from a given identifier such as an IP or email address 2.2.11. Ability to customize and brand with hyperlinks, documents, images 2.2.11.1. Limiting of outside advertisements or branding 2.2.12. Capability to set start and end time for capturing responses
