Specifications include, but are not limited to: The Department of Health and Human Services (Department) is seeking proposals for a suite of Integrated Care Management (ICM) solutions and the implementation services to migrate the Hospitals from their Current Legacy Systems to the awarded Bidder’s ICM Solutions as defined in this Request for Proposal (RFP) document. This document provides instructions for submitting proposals, the procedure and criteria by which the awarded Bidder will be selected and the contractual terms which will govern the relationship between the State of Maine (State) and the awarded Bidder. The Department’s implementation of the awarded Bidder’s ICM Solutions shall support patients receiving Medicaid coverage as well as patients having other insurance carriers. The implementation will be funded jointly by the federal Centers for Medicare & Medicaid Services (CMS) and the State. The goal of the implementation is to improve and enhance care coordination within the Department’s Hospitals and to potentially leverage the implementation to also support the Department’s Public Health Nursing (PHN) program. All deliverables and services related to the awarded Bidder’s ICM Solutions, hosting and operations shall be subject to both State and federal requirements. Through this ICM RFP, the awarded Bidder shall migrate active inpatient and outpatient records from the Department’s Legacy Systems to a new suite of ICM Solutions.
