Specifications include, but are not limited to: Create, operate, and maintain a “Post-Permanency Services” website that contains curated information that must be relevant, easy to access, and easy and valuable for Idaho’s post-permanency population to consume. The website must include general information, contact information to facilitate access to available statewide services, and any other resource sets (services, people, locations, information, research, and skill building/on-line tools) that could help improve the quality of life for a post-permanency family. Offer statewide support to post-permanency families. The process of providing support must include Universal Services, Selective Proactive Outreach Services, Indicated/Assessment and Referral Services, and Intensive/Case Management Services to families. (Attachment 6 – Definitions) The focus is on outreach and engagement with families to enhance their willingness to participate in early post-permanency services, reducing the risk of post-permanency disruption. Provide Post-Permanency Assessment & Referral Services for all qualifying families applying for this service, regardless of the varied lifestyles, backgrounds, languages and ethnicities of families. To qualify a family must have obtained guardianship through a foster care case or adopted through a foster care, private, independent, or intercountry adoption in any state (excluding step-parent or birth parent adoptions). The Contractor is responsible for interpretation and translation costs. Use of the Department’s language line will be available.
