Specifications include, but are not limited to: A. A logbook will be kept on-site for each piece of equipment to be maintained by the Contractor. The logbook shall contain the date each semi-annual maintenance was performed. B. The frequency of semi-annual maintenance shall be no more than one hundred eighty (180) days between services with the first service to be conducted within thirty (30) days of contract execution. C. Contractor will be required to contact the Area office On-Site Contact or Designee and make an appointment to perform the semi-annual maintenance at least three (3) days in advance. Contractor will check in with the Area office On-Site Contact or Designee upon arrival and check out when the work is completed. If there are any discrepancies on invoice concerning hours worked, the CHP will refer to sign-in sheet to confirm actual hours at facility. D. The Scope of Work included in this Agreement will serve as a check-list for the HVAC Maintenance Services performed. Contractor will be required to bring the check-list each time the semi-annual maintenance is performed. All items on the check-list will be checked off as performed. Upon completion, Contractor will provide the Area office OnSite Contact or Designee with the check-list. The check-list will be signed and dated by both Contractor and the Area office On-Site Contact or Designee and filed in the appropriate logbook. Additionally, an extra copy shall be left with the Area office. This check-list shall accompany the invoice prepared by Contractor for the semi-annual maintenance
