Specifications include, but are not limited to: This is an outcome based proposal to provide prevention services to juvenile clients referred for services by the Luzerne County Juvenile Probation and Children and Youth Services. The proposed program should include services to children and families, utilizing evidenced based therapy taking into account issues identified in the Youth Level of Service Assessment (YLS). The program will also address areas identified in the YLS but also work intensively with the families to address areas such as parenting, career guidance, and other areas of concern/resource coordination. Parents will be actively involved in the process. Utilization of natural supports to assist the youth with adaptive skills and functioning within their environment are additional efforts. Services are to be available multiple times per week, during day and evening hours and delivered in accordance with the client’s risk/needs. The program should prevent referrals to more expensive traditional services. Staff should develop linkage with MH/D&A services and ensure juveniles have rapid access to care, continuity of care and comprehensive discharge plans. The provider will understand criminogenic needs through provided assessments. They will collaboratively work with County Staff to develop and assist juveniles in achieving case plan goals. The primary goal is to provide a program to reduce the probability of juveniles re-offending and to promote their successful achievement within the community.
