This is an outcome-based proposal to provide prevention services to high risk juvenile clients adjudicated delinquent/dependent and referred for services by the Luzerne County Juvenile Probation and Children and Youth Services. The proposed program should include social skills training, parenting education, study skills, career guidance and appropriate interactions with authority figures. Parents will be actively involved in the process. Utilization of natural supports to assist the youth with adaptive skills and functioning within their environment are additional efforts. Services are to be available multiple times per week, during day and evening hours and delivered in accordance with the client’s risk/needs. The program should prevent referrals to more expensive traditional services. Staff should develop linkage with MH/D&A services and ensure juveniles have rapid access to care, continuity of care and comprehensive discharge plans. The provider will understand criminogenic needs through provided assessments. They will collaboratively work with County Staff to develop and assist juveniles in achieving case plan goals.
