This project is designed to afford Intermediate Units (IUs) across the Commonwealth of Pennsylvania an opportunity to evaluate and enhance transition programming through data-driven decision-making. Over the course of two years, the grantees use a researched based selection process (see Appendix) to screen and assess all local education agencies (LEA)s in their region. One LEA will be selected based upon the highest need according to the score from the assessments. The grantee will collaborate with the selected Local (LEA) to implement evidence-based practices, and foster systems-level changes that significantly improve the transition planning and outcomes for students with disabilities within the selected LEAs School District. The project goals include: 1. Build and support teams to improve outcomes for transition-aged students with disabilities using data to identify and implement evidence-based practices. 2. Provide support to school-based teams to improve transition services and supports to transition-aged students with disabilities. 3. Empower educational leaders with competencies needed to support systems-level change that improves transition planning. 4. Build the capacity of teams to implement practices that engage parents and families as meaningful partners in the transition planning process. 5. Provide tools and support to school-based teams to increase cross-agency collaboration efforts. 6. Enhance youth and family engagement in transition planning, programs, and services through the implementation of the LEA’s transition systems framework, as evidenced by 85% of survey respondents reporting increased participation in these activities on the implementation progress and impact survey.
