Specifications include, but are not limited to: Delaware County Department of Human Services is requesting proposals for a ten-bed residence that offers supportive housing for older adults with a serious mental illness and allows them an opportunity to age in place. TheDepartment of Human ServicesMental Health Office isseekingaprovidertodevelopandimplement a supportive housing residential program for older adults with severe mental illness and co-occurring disorders. Addressing the intersections of accessible housing, behavioral health, physical health and home health, the provider is tasked to ensure that chronic and emergent needs of residents are met. The program will be developed to address Medical Assistance eligible HealthChoices residents who are age 50 and over and have a severe mental illness and a physical health diagnosis. Residents may have co-occurring needs such as substance use disorders, a developmental disability or be medically fragile. Eligible individuals, not yet nursing home eligible but do require additional supports to maintain continued community tenure and a sense of independence. This program will provide behavioral health staffing while collaborating with physical health insurers for medically necessary treatment. These physical health services could be provided in or out of the proposed residential program. The Department of Human Services, Mental Health office is interested in receiving proposals that can achieve the following goals: • Supporting the recovery and wellness of individuals in their own living environment. • Facilitating ongoing community engagement and achievement of personal goals by helping individuals participate in behavioral health services. • Providing support to individuals while they “age in place.” • Support/assist persons to receive needed and recommended healthcare services (i.e. specialists, recommended annual screenings, oral health, and home health assistance). • Support overall needs for nutritional health and safety. • Coordinating with the Delaware County Office of Services for the Aging (COSA) for services (aging waiver, options program, Community HealthChoices, etc.).
