Specifications include, but are not limited to: Youth / Young Adult Coordinator – Healthy Transitions The Youth / Young Adult Coordinator will be responsible for working with the Project Director, the County Leadership Team, and local county youth / young adult-serving systems on implementation of the proposed Healthy Transitions grant goals (Youth Revolution Initiative). The duties include: Working with the identified population community partners and other leaders in the two counties, along with the State. The ability to collaborate with the grant’s evaluation team, including the ability to implement the Everyday Leadership Skills and Attitudes assessment. The ability to provide peer support when needed. The ability to assist in designing and implementing policies, procedures, and outreach strategies to successfully engage the target population. Demonstrated ability to work with individuals from diverse backgrounds in a trauma-informed, culturally competent manner. The Youth / Young Adult Coordinator will work closely with the Evaluation Team. Family Peer Support/Lead Family Coordinator – Healthy Transitions The Family Peer Support / Lead Family Coordinator will be responsible for working with the Project Director, the County Leadership Team (CLT), and local county child-serving systems on implementation of the proposed goals of the Healthy Transitions Initiative. The duties include: Working with the CLT to develop the strategic plan, working to implement the goals of the strategic plan, working with youth / young adults and families to develop support and advocacy organizations, building on existing resources such as local Mental Health Associations and National Alliance for Mental Illness (NAMI), and assisting in the recruitment of additional Family Members to be part of the CLT. This position will also offer peer support and/or advocacy to parents and caregivers with youth and young adults involved service systems. The Lead Family Coordinator will work closely with the Evaluation Team. Peer Specialist Services (Certified) – Healthy Transitions The Peer Specialist acts as a role model for individuals receiving mental health services and works to form relationships with consumers in order to better understand and communicate needs pertaining to recovery goals and developing community supports. The Peer Specialist will be responsible for working with individuals that are accepted to any of the identified population (youth / young adults ages 16-25). The duties include, but are not limited to: working with the existing services to develop a plan for the identified member, working with youth and families to develop support and advocacy for all involved in the program, building on existing resources such as local Mental Health Associations and National Alliance for Mental Illness (NAMI), and assisting in facilitating group sessions for members who have advanced in their recovery.
