The purpose of this cooperative agreement is to establish a partnership between Youth Advocate Programs (YAP®), Inc. and the qualified and awarded vendor to: Reduce out-of-home placements and congregate care in the jurisdictions awarded. For Child Welfare: Expand child welfare reunification and navigation services delivered through the YAPWrap® service model including youth transitioning out of care. For Youth Justice: Expand support of emerging adult youth justice populations ages 16-25 through YAPWrap® as an alternative to incarceration or congregate care. Increase youth and young adult self-sufficiency, economic well-being and permanency. Support youth/young adult and family safety and well-being. Improve support for students in public schools who have behavioral, academic learning loss, trauma, truancy or other high/complex needs and improve parent/guardian engagement. Conduct research and evaluation on pilot programs goals and outcomes. Develop a sustainable program through local funding sources at the end of the pilot program period.
