Specifications include, but are not limited to: A. Participants to be served by the Community Partner: A participant eligible for the training component of the requested Services must meet the following characteristics: 1. Age group: 18 to 55 (i.e., at least 18 at the time of enrollment); 2. A resident of public or assisted PHA housing, including alternative management entity sites (proven by current lease); 3. Have High School Diploma or GED; 4. Participants with a GED must have reading/math levels of at least 8th grade; 5. Seeking placement in a post-secondary institution, apprenticeship, internship or advanced training leading to viable employment and self-sufficiency. CHWs trained through this program will subsequently provide community health programming and services to PHA residents of all ages, especially but not exclusively those managing chronic health conditions. B. Numbers of Participants Served: Proposer must serve: 1. A minimum of 6, maximum 10 participants per cohort across various communities. Targeted communities are North Philadelphia, South/Southwest Philadelphia, West Philadelphia and Northwest/Northeast Philadelphia. 2. Each Vendor should set forth the number of weeks required for each training program as well as the hours worked per week. PHA anticipates training and employment happening simultaneously. C. Technical Requirements: Proposer’s operational plan should address the following: 1. Staff-to-student ratios, as prescribed by the Pennsylvania Common Core Standards, must be attained. Child abuse clearances and criminal background checks must be secured and evidence provided for all staff and volunteers working directly with participants; 2. Obtain and submit a certificate of insurance for liability insurance for the space being used, and coverage which offers protection against bodily injury claims brought by persons participating in covered activities. D. Location of Services Options The program will be held at facilities within the Philadelphia County (leased or owned by Vendor and/or subcontractor). CHWs will be expected to perform duties in the field and at various PHA properties. The Proposer’s program should include training, employment and options for continued professional development while on the job. E. Enrollment and Retention: PHA shall provide the Community Partners with lists of PHA resident participants that will be suitable for enrollment in the programs. The Community Partner does not need to recruit independently but must coordinate with PHA to ensure the proper number of participants are enrolled and attend classes. 1. Continuity and focus on the participant’s part is critical for long-term success. Each Proposer must: a. Describe in detail your strategy for enrollment, and orientation of the participants who will participate in this program. Describe what role each partner will play in this process. The Proposer shall provide monthly reports to PHA detailing its retention figures. The Community Partner must supply sign in sheets that indicate time of sign in and sign out of each participant. b. Describe how you will work with the duly elected Resident Council at each site(s). c. Describe how your program will adjust to delays or postponement 2. Proposer shall provide recruitment presentations to residents at each location prior to each program. 3. Proposer shall meet with PHA Program Administrator to discuss recruitment and efforts to retain the participants in the program. They shall discuss problems regarding retention and possible solutions. 4. PHA shall monitor the verification of participant recruitment and retention and may withhold payment upon a preponderance of evidence that the Community Partner failed to exhibit good faith in both recruitment and retention of participants. 5. Proposer will include or invite a PHA representative(s) to all individual or group meetings that involve a resident or resident’s performance. 6. Community Partner may offer SEPTA transportation passes to participants for their transportation to and from the classes which may be reimbursed, for the exact cost of the transportation passes from SEPTA, upon invoicing to PHA. Vendor should include in proposal pricing any proposed costs for which it may seek reimbursement.
