Specifications include, but are not limited to: • Conduct intake to assess the needs and eligibility of individuals and families applying, maintain a waitlist of those eligible for the program • Facilitate written agreements between host sites and guests • Enforce program rules and guidelines and problem-solve conflicts that may arise • Provide case management for the guests that enter the program, coordinating supportive services and referrals including Community Connect • Collect and enter data into ServicePoint (HMIS) • Support city efforts in neighborhood communications and relationship building • Visit the host sites to maintain regular supervision and identify any additional needs that guests or sites may have
