Specifications include, but are not limited to: 1.2.1 Contractor shall provide audiometric testing for only those personnel enrolled in the Oregon State Police's hearing conservation program as designated by the OSP Health & Safety Manager. OSP shall provide a complete list of OSP law enforcement and forensics firearms testing personnel, testing locations, and employee worksites to the Contractor. The OSP Health & Safety Manager is responsible for updating this list. 1.2.2 All audiometric tests shall be conducted by a licensed or certified Audiologist, Otolaryngologist or by a technician who is certified by the Council of Accreditation in Occupational Hearing, as specified under "Qualifications of Audiometric Contractor" per OAR437-002-0095 and 29 C.F.R. 1910.5, as adopted by OAR 437-002-0005. 1.2.3 Audiometric tests shall be pure tone, air conduction, hearing threshold examinations with test frequencies including 500, 1000, 2000, 3000, 4000, 6000 Hz. Pulsed-tone and self-recording audiometers, if used, shall meet the requirements of OAR 437-002-0095 and 29 C.F.R. 1910.5, as adopted by OAR 437-002-0005.
