Specifications include, but are not limited to: Family Caregiver Support Program Case Management is a comprehensive service provided to Family Caregivers who are caring for older adults, or are caregivers of a relative child or children, or are caring for a loved one with Alzheimer’s disease or other dementia. This program also covers non-parental relatives age 55 or older who are caring for a related adult with a disability, or a relative child 0-17 years old. Contractor shall provide case management services including: assessing needs, developing care plans, documenting in Oregon Access, authorizing services, coordinating services, counseling and problem-solving, evaluation of services, advocacy, follow-up and reassessment. This category includes the Star Caregiver intervention. The definition of Family Caregiver includes friends, neighbors and domestic partners who care for someone age 60 or older, or of any age if the person in care has Alzheimer’s or another dementia. Case managers for family Caregivers can access ADVSD’s Family Caregiver Support Program, which provides small grants to family caregivers. Note: Peer support groups for family caregivers are an aspect of Focal Point. This event is seeking proposers to specifically support the Latinx and/or Native American populations throughout the County. The County seeks to support both populations with a culturally specific provider.
