DHS’ purpose for accepting Applications for vendors for long-term and short-term Contracts or IGAs for CME Services is to ensure continuity of care to Individuals when a CME is planning to end their Contract or IGA with DHS; DHS is choosing to end the existing Contract or IGA; or DHS has identified a specific area needing assistance. These CME Services, at a minimum, will be: a. Deliver Case Management Services, as described in OAR Chapters 411 to 415, to Individuals who are eligible for and desire Case Management from a CME. Case Management Services must be person-centered and self-determined by the Individual. b. Provide Services that meet the basic management, programmatic, and health, safety, and human rights regulations in the management of a CME for Individuals with I/DD. c. Ensure that eligibility for Services is determined in accordance to OAR 411-320-0080 or OAR 411-340-0100.
