Specifications include, but are not limited to: Access to Medical Care: x Anticipated service population of at least 170 unduplicated individuals will be screened and/or referred to community-based mental health, dental or medical services annually, which include but are not limited to: o Oregon Health Plan (OHP) and Women, Infants, and Children (WIC) o Lane County Behavioral Health or other behavioral healthcare providers o Physical/medical healthcare providers Performance Outcomes: x 100% of eligible low-income Latinx households will be enrolled in applicable healthcare benefits. Access to Mainstream Programs and Benefits: x Anticipated service population of at least 630 unduplicated community members (individuals) will be screened for mainstream program and benefit eligibility and supported in navigating and accessing community-based programs and supports annually. x Mainstream programs and benefits include but are not limited to: Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), Veteran’s programs, and other Department of Human Services (DHS) self-sufficiency programs.
