• The contractor shall provide hauling from the point of origin (City of Salem “City”) designated locations to their facility by a licensed carrier. The contractor shall perform/provide all required manifests. Once the materials leave the point of origin, ownership is transferred to the Contractor. • The quantity of each type of material accepted for recycling shall be recorded and accurately reported to the City. • The materials accepted for recycling are to be transported by a licensed carrier to an appropriate, permitted facility for recycling. • The contractor shall maintain complete “cradle-to-grave” chain-of-custody documentation, as required by law. The contractor shall submit to the City a certification that the materials were properly recycled. • The City is responsible for ensuring that the contractor has reasonable and safe access to the containers of material for loading purposes. • Contractor shall provide copies of the professional certifications to perform the waste disposal services required within this bid. • The contractor must provide a copy of their ID number and permit to transport, treat, recycle, store, or dispose of the wastes included in this bid. • Contractor shall be knowledgeable of forms and certifications for waste disposal and any specific material to the performance of this contract. • Contractor shall have an adequate past record of performance, verifiable through references. List of minimum of three (3) references shall include: business name, contact person and telephone number on the attached References Form, which is required to be returned with your bid. Failure to provide the References Form with your bid shall result in rejection of your bid. Include name and location of governmental or commercial entities for whom the services were performed and what the specific services were. • Contractor shall describe the amount of involvement required by City staff. Be specific about the number of hours per month and skill level of individuals who should be involved. • Contractor shall describe pick-up schedule for the Salem area and turnaround time. • A fee schedule of all classifications of waste materials assigned to this contract. The proposal shall include all costs necessary to perform the required Treatment/Storage/Disposal (TSD) listed in the scope of required services. • The cost proposal shall include a separate lump sum fee (total cost to City) for each item of the Cost Proposal Breakdown. Such fees will be utilized as not to exceed contract amount. The general rate schedule shall include but not be limited to the following: o Provide hourly rates (including additional charges for “Personal Protective Equipment” (PPE), direct salary, overhead and other related costs) for all personnel anticipated to be utilized for this contract and other pertinent unit charges. Such hourly rates shall be complete, all inclusive costs to the City. o Provide hourly rates for overtime, emergency “call outs” and holidays. Include the expected response time in these situations. o Provide hourly rates for equipment used. o Charges for materials including but not limited to drums, overpack drums, PPE, spill materials, labels, sample containers, sampling equipment, boxes, containers, and pallets. o Fees charged to profiling, manifesting, labeling, lab analysis, pick-up charges, freight charges, transportation, stopping, loading, fuel, energy, security, mobilization, and other surcharge fees.
