Urine Collection Kit Contents 1 90ml specimen container, with temperature indicator strip pre-affixed to the side, in a sealed bag. 1 Specimen container label with matte finish. 1 Self-sealing biohazard transport security pouch that includes an absorbent sheet. 3 1 ¼ x 7 in strips of red tamper evident tape, printed with “EVIDENCE” and space to write initials. 1 Procedural instruction for collecting urine sample (Text provided by OSP). 1 Mailing box approximately 5” wide, 3 ¾” deep, and 2 ½” high. Contents of kit must fit inside mailing box and be entirely shrink wrapped with clear plastic. Mailing container must be designed with only one opening and must not tip easily when stacked. Each box must be labeled on both ends. The labels must be at least 1” x 2.5”, have a green border, and contain the following in green ink: “Urine Collection Kit”, the reorder #, and the lot #. Labels shall be placed centered within ¼” of the bottom of each end of the box. (Visual examples in attachments tab of the solicitation). Blood Collection Kit contents 21 gauge blood collection needle with safety mechanism to reduce the risk of needle-stick injury and exposure to blood-born pathogens.
