The purpose of this contract is for a Contractor to provide mobile audiometric testing and counseling services as defined in this contract for ODOT Regions 1, 2, 3, 4 and 5. Contractor shall provide audiometric testing for Agency staff at ODOT regional testing sites and locations as scheduled in coordination with Contractor and Agency. Contractor shall conduct and review approximately 1000 audiograms per year for Agency Personnel. Testing sites and locations include, but are not limited to, the following and may include day and night shift crews, employees working four-day shifts where Tuesday – Thursday testing is optimal and where early morning testing is needed: Region 1 Clackamas, Cascade Locks, Sandy, Government Camp, East Portland, Troutdale, Sylvan/Baldock. Region 2: Astoria or Warrenton, Banks, Clatskanie, Manning, Tillamook, Florence, McMinnville, Rose Lodge, Seal Rock, Salem, Albany, Detroit, Eugene, Springfield, Corvallis, McKenzie Bridge, Santiam Junction, Oakridge, Veneta. Region 3: Ashland, Central Point, Coos Bay, Grants Pass, Gold Beach, Prospect or Steamboat, Roseburg, Yoncalla or Reedsport. Region 4: Bend, Klamath Falls, La Pine, Lakeview, Moro, Prineville, The Dalles. Region 5: Baker City, Burns, Canyon City, Enterprise, Heppner, Hermiston, Jordan Valley, La Grande, Ontario, Pendleton, Meacham.
