Specifications include, but are not limited to: C.2.1. The supplier shall provide a complete turn-key operation to include all waste monitoring, labeling, marking, loading, placarding, manifesting, training, transportation, and disposal. Disposal must meet all applicable federal, state and municipal regulations, laws and ordinances as defined below in Oklahoma State Department of Environmental Quality Regulations Governing Management of Solid Waste. C.2.2. The Supplier shall pick up Biomedical Waste from each County Health Department site on the attached list, Attachment 1. The OSDH reserves the right to add or delete sites from this list as necessary and in writing to the Supplier with a 30 day notice. C.2.3. Pickup shall be on scheduled at mutually agreed upon time to both parties, and shall be at a site designated by the Oklahoma Statement Department of Health. See Attachment 2 for the requested frequency, and any special conditions for, each site. The OSDH prefers pick up services to be performed within the normal operating hours of each location. County Health Department personnel shall not, under any circumstances, be responsible for transporting waste to another location. C.2.4. Estimated usage for purposes of this bid shall be 3200 boxes/job per year. This estimate is for evaluation purposes, and is not a guarantee of usage. The contract is for an indefinite quantity. C.2.5. The bid price shall be expressed on a per box basis and shall include all costs associated with performance of the contract, but not limited to: supplies, pick up, transportation, disposal, shipping, fuel, etc.
