Safety footwear for employees shall meet the requirements and specifications set forth in American Section of the International Association for Testing Materials and Occupational Safety and Health Administration Standards: o ASTM F2413-24 Standard Specification for Performance Requirements for Protective (Safety) Toe Cap Footwear o ASTM F2412-24 Standard Test Methods for Foot Protection o OSHA Standard 1910.136 Personal Protective Equipment Foot Protection Certain job classifications allow for low-cut, mid-top, high-top, or athletic safety shoes (basically leather) to be used. It is the responsibility of the City Department to ensure that employees are buying the proper safety footwear for their job classification. For the purposes of this bid, footwear classifications are defined as follows: Athletic and work shoes with a height of less than 4 inches are considered low-cut. Mid-top work boots, including hiking boots, typically range in height from 4 inches to 8 inches. High-top athletic shoes generally do not exceed 6 inches in height, while high-top work boots are defined as having a minimum height of 9 inches or greater. Employees are required to present an authorized Form TUL-4084-A Safety Footwear Requisition when acquiring safety footwear. The employee must show their City Employee ID badge to validate they are in fact the employee named on the Form TUL-4084-A. Seller must retain the white copy of Form TUL-4084-A. Seller shall make Sales usage reports for the Safety Footwear Program available upon request from City of Tulsa.
