1. Scope of Services The City is requesting Bids for a Prescription Safety Eyewear Program for use by the Human Resources Department. Delivery Requirements All prices quoted shall be based on delivery F.O.B. Tulsa, Oklahoma or to any other points as may be designated in the Specifications, with all charges prepaid by Seller to the actual point of delivery. Bids must state the number of Days required for delivery under normal conditions. Absolutely no items over 500lbs. may be directly shipped to the City requiring the City personnel to unload from the shipping truck or trailer. The City carries no responsibility for any damages incurred during an unloading occurrence. All unloading shall be the responsibility of the Seller and/or shipping entity. 2. Specifications The Bid must meet or exceed the following Specifications. GENERAL: Bid prices must be net, including transportation and handling charges fully prepaid to destination(s) within the City of Tulsa. The City does not guarantee any specific quantities to be purchased from this Agreement. As of this time, the City has approximately 250 employees that need prescription safety eyewear. DESCRIPTIVE LITERATURE, BROCHURES, SPECIFICATION SHEETS: Bidders must provide descriptive literature, brochures, and/or specification sheets on the items bid. This information is to be part of Your Bid and included with Your Bid Package. FAILURE TO COMPLY WILL RESULT IN REJECTION OF YOUR BID. Prescription Safety Eyewear: The prescription safety eyewear for employees (approximately 250) shall meet the requirements and specifications set forth in the American National Standard for Occupational and Educational Personal Eye and Face Protection Device, (ANSI) Z87.2-2020 and CFR 1910.133 or CFR 1926.102. Type of Material: Prescription safety eyewear shall have frames that are high impact; tight fitting, plastic only (no wire frames), integrated side shields, non-electrical conducting, and the lenses will be made of poly carbonate; also include styles that do not have metal parts and made of a nonconducting substance but still meet the current ANSI requirements, called dielectric safety glasses. Safety eyewear will be available with the following options: • Lens Style – single vision, bifocal, trifocal, progressives. • Tint and Coating – multiple tints, photochromic, transitions, polarized, super-cote, anti-reflective, UV, and anti-fog. • At least 10 frame styles to choose from, that are not dielectric safety glasses. • At least two (2) frame styles to choose from that are dielectric safety glasses. • Provide the option of either a hard or soft case, with or without a belt/pocket clip, and appropriate lens cleaning cloth with each purchased pair of prescription safety eyewear.
