Contractor shall provide a firm, fixed-price to furnish and install a water service line and road bore 371108 US Highway 62, Okemah, Oklahoma 74859. Latitude: 35.476892 (N), Longitude: -96.395457 (W). In accordance with the Statement of Work. This is a new home so the address may not work in your GPS. Use the latitude and longitude Latitude coordinates.
The period of performance for this requirement is 60 calendar days from Notice to Proceed.
Site visit is scheduled for January 19, 2023 at 10:30 AM. Questions are due no later than January 26, 2022 5pm CST. The closing date for receipt of quotes is February 10, 2023 COB CST. The offer shall be received by e-mail to jeffrey.earnest@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address, no later than 5:00 p.m. CST, on February 10, 2023.

THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING:
A) Firm, Fixed price quote with attached bid sheet
B) Technically specifications of the statement of work, acceptable materials list to be used.
C) (2) Relevant past performance of this type of work, shall provide organization’s name, point of contact, summary of project and award amount
D) Bid bond form (SF24) attached is required at $35,000. If over $35,000 and selected as the acceptable vendor proposal, performance (SF25) and payment (SF25A) bonds will apply
Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable.
Evaluation shall proceed as follows:
1. Quotes shall be ranked by price, from low to high
2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote
3. This process shall proceed until a technical acceptable quote is identified

CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS.
The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
