2. B01 Attachment J05 CIH As-Built SW section A-4_08-09-2023 replaces the prior version. The updated attachment highlights
the room to be renovated.
3. All other terms and conditions remain unchanged.
Request for Proposals: Claremore X-Ray Room B Renovation
Goal: The objective of this project is to remodel X-ray Room 342 in the Radiology Department to accommodate a new x-ray unit at the US PHS IndianHospital located in Claremore, Oklahoma.
Location: 101 S Moore Ave, Claremore, OK 74017
Period of Performance: 90 days
Construction Magnitude: In accordance with FAR 36.204, the magnitude of this construction project is anticipated to be between $250,000 and $500,000.
Type of Contract: The Indian Health Service intends to award a single Firm Fixed Price (FFP) contract in support of this requirement. This Request for Proposal (RFP) is issued in accordance with FAR Part 36 procedures with Lowest Price Technically Acceptable (LPTA) evaluation criteria.
