Specifications include, but are not limited to: Division of Emergency Medical Service (EMS) Headquarters cleaning services, facility 1 lobby area, 6 Restrooms, 1 cafeteria, 1 Gym Area and 7 Hallways Weekly Cleaning Service to be provide every Wednesday between 7 am to 7:30am to begin before business hours begin. Service will be performed on the Tuesday prior if Wednesday is a holiday; (service cancelled week of THANKSGIVING AND CHRISTMAS). Please see below specifications. 1. Vendor must provide adequate staffing to finish weekly and monthly cleaning in a 2 4- hour period 2. Services will be performed on the same day of the week determined by the Division of EMS 3. Vendor shall be responsible for supplying all cleaning agents and equipment needed 4 Vendor must fulfill all liability insurance requirements 5. The Division of EMS will supply all toilet paper, paper towels, hand soap, and trash bags 6. Purchase order will not exceed 15,000.00 for services provided. Item 1. Services that shall be completed weekly. A. removal of all trash from trash cans and replace liners. B. vacuum all carpeting and mats throughout areas listed. C. sweep and mop all linoleum floors throughout areas listed. D. wash and disinfect all lavatory fixtures, sinks, bowls, and urinals interior and exterior in all restrooms, replenish all toiletries as needed. E. wash and disinfect cafeteria sink and counter tops Item 2. Monthly cleaning services. Services that shall be completed monthly. A. clean interior and exterior glass entrance doors. B. Clean Front Lobby and Cafeteria windows, windowsills, and blinds. C. Clean and disinfect all cafeteria furniture, appliances, and water fountain D. Clean and disinfect all restroom partitions, doors and light switches Item. *Please provide pricing based on weekly rate. Taking into account the weekly and monthly services.* General Liability and Automobile Liability Insurance: The successful bidder shall be required to take all necessary precautionary measures and to perform all work required for this contract in such a manner as to adequately protect people and safeguard property and existing facilities for any damage due to its operations. Any such damage shall be satisfactorily replaced or repaired by the successful bidder at its own expense. A. Insurance Requirements The successful bidder shall purchase and maintain during the term of the contract general liability insurance including but not limited to personal, injury, property damage, contractual, liability, owners' and contractors' protective liability and products/ completed operations coverage wherein the City of Cleveland is named as an additional insured. Special hazards such as business automobile liability insurance are addressed in Section C. Coverage shall protect the successful bidder contractor and any subcontractor performing work under this term of the contract from claims for damage for personal injury, bodily injury, including accidental death, as well as for claims, for property damages with may arise from operations under this contract, whether such operations be by the successful bidder, or by any subcontractor or by anyone directly or indirectly employed by any of them. An original certificate of insurance and a copy of the additional insured endorsement naming the City of Cleveland as an additional insured shall be submitted with the bid. Such documents shall be as to form coverage, carrier, and limits satisfactory to and approved by the Director of Law. The additional insured coverage provided the City under contractor's insurance policy(ies) shall be primary with respect to contractor's general liability.
