SPECIFICATIONS: On-line Un–Numbered Registration Renewal Form with Alternative Decals A. SIZE: Forms: Carrier sheet shall be 8-1/2” x 11” to be “C” folded on a Folder/Inserter by the agency Renewal forms: each decal shall be numbered consecutively in black ink in Helvetica or similar typeface no smaller than ¼” in height in the blank spaces provided. . Decals: Decals shall have a plate size of 3" x 6" A single decal to be rectangular and must be die cut. B. STOCK: Forms: Carrier sheet shall be 28# White Premium #4 OCR Bond. Decals: Shall be Shall be Tampermark PM 200 White Void II TC-390 with adhesive back that will expose the wording “VOID” when removed after initial application. Decals are to be at least 1 mil in thickness and will have a 1 mil over laminate to protect the printing and the decal from variable weather conditions, abrasion, UV radiation and cleaning agents.
