1.0 OFFER SUBMISSION All offers must be submitted in electronic format to the Contract Specialist, by the date and time specified in Block 8 of the SF 1449, via email to Michele.Bartelt@va.gov All offers must be viewable using Microsoft Office products or Adobe Reader. Hard copy offers or offers submitted by fax will not be accepted. Section A - Offer Form (Standard Form 1449) Signed and dated, SAM Unique Entity Identifier (UEI) shall be entered in the block with the Offerors name and address. Section B Pricing Pricing shall be submitted as requested on the Schedule of Services. The total price will be evaluated by the Government. Proposed price must reflect a clear understanding of the requirements and be consistent with the requirements and is consistent with the various elements of the offer. Section C Offerors Representations and Certifications A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at www.sam.gov/portal/public/SAM/. All Offerors shall be registered and current in the System for Award Management (SAM) database at time of offer submission. Contractors claiming Service-Disabled Veteran Owned Small Business or Veteran Owned Small Business size status are required to be CVE verified and listed in the Vendor Information Pages (VetCert) upon the solicitation closing date/time. Verification instructions can be located at https://www.vip.vetbiz.va.gov/. This statement is only required for solicitations being issued with a socioeconomic set-aside for Service-Disabled Veteran Owned Small Business or Veteran Owned Small Business. See the SF1449 and/or Contract Clauses for set-aside information. Section D Technical Approach Offerors shall submit a quote that addresses all aspects of the Statement of Work (SOW). The offeror shall identify any subcontractors proposed under this solicitation. All technical elements applicable to the utilization of subcontractors must be addressed in the technical quote and detail provided. The Technical Volume shall not exceed twenty-five (25) pages in length (minimum 12-point font and no less than ½ inch margins). Page limitation includes drawings, charts, images, etc., and excludes section dividers, table of contents, glossary of terms, cross-referencing indices, and resumes. If more than 25 pages (not counting excluded pages) are submitted, only the first 25 pages will be considered. 3.0 AWARD BASIS Note that FAR Subpart 13.5 Simplified Acquisitions Procedures for Certain Commercial Items procedures will be used. The Federal Government intends to award a Firm Fixed Price Contract to the responsible offeror whose offer, conforming to the requirements in this request for quote, will be most advantageous to the Federal Government, price and other factors considered. Award will be given to the offeror which provides the best value in accordance with the evaluation criteria in E.6. Offers will be evaluated for the following: Price Technical Capability: Contractor shall provide proof of compliance with all aspects of the Statement of Work If any offer has an evaluation factor that is determined unacceptable, the offer shall be considered unacceptable. TECHNICAL EVALUATING RATING DEFINITIONS: Outstanding: Offer meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses. Risk of unsuccessful performance is very low. Good: Offer meets requirements and indicates a thorough approach and understanding of the requirements. Offer contains strengths which outweigh any weaknesses. Risk of unsuccessful performance is low. Acceptable: Offer meets requirements and indicates an adequate approach and understanding of the requirements. Strengths and weaknesses are offset or will have little or no impact on contract performance. Risk of unsuccessful performance is moderate. Marginal: Offer does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The offer has one or more weaknesses which are not offset by strengths. Risk of unsuccessful performance is high. Unacceptable: Offer does not meet requirements and contains one or more deficiencies. Offer is not awardable. Neutral: Neutral rating is used only for Past Performance Rating. If no relevant past performance has been established by the offeror, a neutral rating shall be assigned.
