The response to this bid should include implants for Occipital, and Cervical spinal fixations (both anterior and posterior spine), lumbar spinal fixations (anterior, lateral, endoscopic and posterior), and sacral/iliac spinal fixations. The spinal bid will consider artificial discs, rods, pedicle screws, locking screws, rod templates, and cross links. Corpectomy cages, TLIF cages, PLIF cages, Bone growth stimulators, 1 level, 2 level, 3 level etc. plates and screws, drill bits laminoplasty cages, staples, and adjustable rods. Lateral spinal, endoscopic spinal and titanium cages will be considered in the bid. Also include trauma products, if applicable. Vendor sets shall have inventory control sheets before sterilization. Vendor’s implants and instrumentation are case representative’s responsibility preoperative -intra-operative, and post operatively. The Facility shall not be responsible or liable for vendor instrumentation or implants. Vendor representatives shall be present for surgical procedures. MHS will not assume any rental fees, delivery fees, or restocking fees associated with instrumentation. MHS shall not be responsible for any charges associated with restocking or replenishing implants. Shipping and handling fees shall be the responsibility of the vendor.
