The Cincinnati Fire Department is seeking to establish a comprehensive social services program to assist the Emergency Medical Services (EMS) Bureau in managing patient cases with high-volume EMS use and/or cases where emergency medical attention is not required. The primary goal of this program is to connect identified patients with the appropriate resources and services to address their underlying needs, thereby reducing the frequency of non-emergency EMS calls and improving overall community health. Shall provide social services aimed at promoting the health and well-being of patients assigned by the Cincinnati Fire Department. The services shall include collaboration with the Fire Department Emergency Medical Services (EMS) Bureau to identify and classify patients, identify social determinants, resolve personal and social impediments to care, coordinate care with other community partners of the CFD and City of Cincinnati, and educate and inform the uniformed members of the CFD.
