1. Patient Identification and Assessment: a. Identify patient needs through referrals and screening processes. b. Maintain case documentation for all contacts with patients. c. Record patient information in computer files as appropriate. 2. Counseling and Intervention: a. Provide counseling regarding follow-up care planning with primary care providers. b. Offer intervention and assistance with basic needs such as transportation arrangements, substance abuse, proper medical care, and homelessness. c. Evaluate and report incidents of alleged abuse. 3. Care Coordination: a. Refer patients to other health care providers and social services as appropriate. b. Maintain contact with other health care providers to coordinate patient care and advocate for patients. c. Ensure patients attend scheduled appointments and participate in follow-up care. 4. Outreach and Education: a. Participate in outreach projects to educate and inform the uniformed members about referring a patient, assessing social determinants, and available health and social services resources. b. Develop learning materials for Fire Department personnel regarding available resources. c. Attend any possible meetings as a representative from the CFD. 5. Data Management and Reporting: a. Document interactions with patients and maintain accurate records. b. Compile and present data and metrics related to patient progression. c. Prepare various reports on operations and activities. d. Monitor and control expenditures. e. Provide feedback to the person(s) who made the referral or who may be involved with the patient. 6. General Duties: a. Ensure proper maintenance and security of assigned buildings, vehicle, and equipment. b. Perform related duties as assigned by CFD supervisors.
