The purpose of this Invitation to Bid (ITB) is to obtain a contractor to provide Pocket Guides for use by the Ohio Department of Health. QUANTITY: 200 (exact quantity). SPECIFICATIONS: SBSP Tooth Selection Pocket Guide, 15” x 11” flat sheet, prints, two sides, four color process bleeds or digital full color, score and folds to 3.75” x 5.5”. Fold in half then three folds accordion folds to 3.75” x 5.5”. There are 10 separate rectangular die-cut windows at folds, 4 (horizontal) rectangular die-cut windows are 3.5” x .125” and 6 (vertical) rectangular die-cut windows are .125” x 5.25”. Both sides are laminated with clear 1.5 mil plastic. A. SIZE: 15” x 11”. B. STOCK: 80# white gloss coated, cover (laminated two sides with clear 1.5 mil plastic); C. PRESSWORK: Shall print offset or digital full color (600 DPI minimum), head to head, bleeds. Shall print in one pass through the press each side. D. INK: Four color process or digital full color. E. BINDERY: Die-cut 10 separate rectangular die-cut windows at folds, 4 (horizontal) rectangular die-cut windows are 3.5” x .125” and 6 (vertical) rectangular die-cut windows are .125” x 5.25”. Laminate both sides with 1.5 mil clear plastic (flush cut), cut to size, score and fold (one horizontal fold) in half and accordion fold (three vertical folds) to 3.75” x 5.5”. F.
