The selected VENDOR will screen, hire, and manage three (3) patient navigators/supporters who will work with eligible residents in the Cuyahoga County Jail. The selected VENDOR will work in partnership with Cuyahoga County Jail administration on training and orientation to the facility, scheduling of services, background checks and other needed and necessary protocol compliance. The selected VENDOR will track services provided and outcomes on a monthly basis to Jail Program Administration. The selected VENDOR may need to be available evenings and weekends to accommodate service provision inside the facility and post-release. The selected VENDOR will outline an appropriate case load number in their response and ensure that caseloads are maximized. The selected VENDOR will hold quarterly meetings with Jail and County Administration to report on work and outcomes, and discuss any challenges and opportunities that arise through the course of the grant period.
