Specifications include, but are not limited to:The purpose of this Request for Proposals (RFP) is to solicit competitive proposals for the delivery of services to eligible individuals and their families under the Help Me Grow Early Intervention Program. EQUIRED SERVICE COORDINATION ACTIVITIES 1. Act as a single point of contact for carrying out activities. 2. Assist parents of eligible children in obtaining access to needed EI services and other services identified on the IFSP including make referrals to providers. 3. Coordinate the provision of EI services that child needs or is being provided. 4. Coordinate evaluations and assessments. Bidders must be aware that current evaluations and assessments are completed by Wood Lane (currently at $24,250 per year) and Wood County Hospital (currently at $4,000.00 per year). 5. Facilitate and participate in the development, review and evaluation of IFSP’s. 6. Conduct referrals and other activities to assist families in identifying available EI providers. 7. Coordinate, facilitate and monitor the delivery of services to ensure services are provided within 30 days of the signed IFSP. 8. Conduct follow up activities to determine that appropriate EI services are being provided. 9. Inform families of their rights and procedural safeguards. 10. Inform family of all activities for which their consent is sought. 11. Coordinate funding sources for EI services needed. 12. Facilitate the development of a transition plan to support a smooth transition of the child from HMG intervention to either pre-school special education services or other appropriate services. 13. Comply with updates, additions and/or corrections to the Ohio Department of Health (ODH) and/or Ohio Department of Developmental Disabilities (ODDD) rules and policies. 14. Assist in answering the grant RFP to ODD for funding including: Narratives, Budgets, Budget Narratives, Program Reports and any other required elements to insure successful grant RFP submissions. (NOTE: The grant RFP is issued through a statewide system in order for the Administrative Agent to receive funds. It is separate from this actual RFP being issued for the program services).
