Specifications include, but are not limited to: Conduct landscape analysis to inform the development of the National Diabetes Prevention Program Pilot. a. Complete Readiness Assessment – Part 1 located on the National Diabetes Prevention Program Coverage Toolkit website. b. Assess Employee Interest and Program Need: i. Assess employee interest and/or needs for delivery of the program, including but not limited to, preferred program delivery mode, preferred program delivery locations (if in-person), program incentives, preferred program day/time etc. ii. Review existing data to determine the number of health plan members that meet program eligibility requirements. Determine the potential costs and return on investment (ROI) o f th e program based on the number of members eligible for the program. This data analysis sho uld include a review of existing de-identified data broken down by race/ethnicity, age, and sex/gender. iii. Submit a summary based on the employee needs and interest assessment, existing data analysis, and potential program costs and return on investment.
