1. Conduct monthly progress update meetings/conference calls with ODH and project team to review data, progress, barriers and successes within the program area. Schedule twenty-four (24) progress update meetings per year by virtual or in-person meetings, with rosters, written summaries and action steps recorded. 2. Provide separate quarterly reports to ODH on progress and outcomes from education. Data compiled will be used to determine an education marketing plan to maximize reach. Webinar planning and participation will include evaluation and sustainability. 3. Create and launch a Virtual Education Series on emergent topics in child health. Recruitment efforts will be targeted in rural, high risk, and underserved minorities areas of Ohio. Topics are subject to change; the order of topics will be based on emergent needs as determined by the offeror and ODH. Each training will be live and be available for enduring credit. The offeror will develop a virtual platform for trainings to be accessed. The offeror will harvest information from attendee feedback and surveys to plan Quality Improvement (QI) topics and format for SFY 27. 4. Provide enduring CME (continuing medical education) and MOC (maintenance of certification) Part II credit for all previous recorded trainings. This will allow for a repository of trainings for providers and professionals to access. Outcomes will be provided to ODH. 5. Develop a digital provider implementation toolkit to be hosted on offerors website. This will house provider specific information for each monthly training. Information will include best practices for topics covered in monthly education. 6. Collaborate with non-profits, community organizations, and internal resources such as practice managers that have a focus on proposed topics areas. Collaborations include trainings, resources, and future iterations of this work. 7. Organize, create, and provide one patient focused digital resource per monthly training. Resources will be published digitally for use by partners engaged in the education program or other providers. ODH to review and approve all resources.
