Specifications include, but are not limited to: 1)Provide direct services and care to Ohio residents diagnosed and living with ALS. Services include anyof the following: a. Wraparound care, including multidisciplinary clinical care.b.Local case management services by health care professionals.c.Distribution of durable medical and augmentative communication devices.d.State and federal advocacy.e.Support groups and patient grants for disease-related expenses.2)Submit an annual program and expense report within 60 days of the end of each State Fiscal Year. Report should include the following information:a. Number of Ohioans served.b. Demographics of individuals served.c. Aggregate program performance data.d.Number and types of services provided.e. Financial summary
