The purpose of this RFP is to solicit Proposals for a Contractor to provide healthcare navigation services to Members. The Agency is seeking to secure a Contractor to increase the quality of care while reducing cost for the State’s Members through effective healthcare navigation services. The Agency is also seeking a Contractor that will offer solutions to simplify access to care, increase engagement in existing programs, increase healthy outcomes, and reduce overall costs for Members and the State.
