The contractor shall provide Cosmetology Services to both male and female patients at Northcoast Behavioral Healthcare’s Northfield Campus. Contractor will utilize standard precautions and provide services according to professional standards. Contractor will be a trained and licensed beautician/barber. The contractor will arrange their own hours with the hospital. Services shall include cutting, shampooing, styling hair, shaving and/or trimming facial hair. Contractor shall company with NBH TB prevention program. • Scheduling for services estimated at 36 hours per week • Fee for Service based billing – No longer accepting an hourly rate as billable unit service rates established in competitive bid. • Limited Services – The only services that NBH would engage in would be the following estimated examples: • a.) Hair cut – Men’s and Women’s b.) Shampoo/Conditioner c.) Shampoo/Conditioner/Cut d.) Beard Trim • Bid prices are required for each service. • Any additional service beyond these could be purchased by the individual patient with treatment team approval. Services billed directly to patient. • Per Performance Criteria will be as follows: Will follow through on schedule 95% of the time. Customers will voice satisfaction with service 92% of the time – Satisfaction Survey.
