Offeror to participate in monthly meetings; communicate with ODH staff monthly regarding the progress and status of each deliverable, successes, barriers, technical assistance (TA) needs and data collection. Submit a written monthly report, including but not limited to: • A detailed description of progress and status of each deliverable. • TA needs, barriers and successes. All progress reports should be submitted no later than two (2) days prior to each scheduled monthly conference call. Offeror shall recruit new healthcare systems/practices and community-based social service organizations into the Unite Ohio network. Offeror will report quarterly on recruitment efforts by submitting written reports that summarize efforts to recruit new healthcare systems/practices and community-based social service organizations into the Unite Ohio network. Information in the reports should include but is not limited to: • Number of and names of new healthcare systems/practices. • Number of new community-based organizations, including organization name and address. • Social needs domains (e.g. housing, transportation, food security, etc.) of new community-based organizations. • Barriers/challenges for expansion. • Relevant changes to the expansion plan submitted in year two.
