STATEMENT OF WORK CONTRACT TITLE: Interstitial Clean Up Maintenance and Service PLACE OF PERFORMANCES: VA San Diego Healthcare System 3350 La Jolla Village Dr. San Diego, CA 92161 PERIOD OF PERFORMANCE: Base: 10/01/2025-09/30/2026 OY1: 10/01/2026-09/30/2027 OY2: 10/01/2027-09/30/2028 OY3: 10/01/2028-09/30/2029 OY4: 10/01/2029-09/30/2030 CONTRACT TYPE: Firm Fixed Price BACKGROUND: To maintain the asbestos containing environment of the interstitial spaces by certified asbestos contractor. Requires a CA 22 License. Hold state and federal certificates to work in an asbestos containing area. Needs Contractor Asbestos Management Course (40 hrs.) and all assigned workers must have asbestos certification of no less than 16 hours with a valid Respiratory Fit Test certification. Certified in Hilti Fire Stop Applications. Knowledge in The Joint Commission EC.02.03.05 reporting requirements. Experience working in a Hospital Environment. Due to the hospital fire safety systems requiring the highest level of confidence in the monitoring and maintenance the following must be considered: 5+ years Knowledge of VASD hospitals, facilities, and systems, 5 + years Hospital Experience must have hospital experience various procedures required to enter different areas. 5+ years of experience with Edwards system and certified Edwards vendor and repair technicians. LASTLY, lowest bidder must meet criteria and have accurate current quote that reflects the required amount of work and pricing. Anything below $200K needs to be questioned as to their understanding of the work required at this location. GENERAL REQUIREMENTS: The contractor shall provide all manner, means, methods, equipment, expertise, incidentals, labor, materials, parts, services, supervision, tools, transportation, and qualified personnel necessary and required to perform the following in accordance/compliance with current code, VA directives, OHASA regulations, industry guidelines / standards, and this statement of work (SOW) throughout the VA San Diego Healthcare System campus. 6.1 Ensure the cleanliness of the interstitial catwalk system by performing HEPA vacuuming, once a month, along all catwalk platforms located within the interstitials of building 1 interstitials. 6.1.1 Ensure the cleanliness of the interstitial mechanical, electrical, elevator, AC, and/or other rooms located within the Interstitials of building 1, by performing HEPA vacuuming once a month. 6.1.2 Ensure the cleanliness of the interstitial catwalk system by performing damp mopping, once a month, along all catwalk platforms located within the interstitials of building 1 interstitials. 6.1.3 Ensure the cleanliness of the interstitial mechanical, electrical, elevator, AC, and/or other rooms located within the Interstitials of building 1, by performing damp mopping once a month. 6.1.4 Ensure the cleanliness of the interstitial catwalk system by performing a wet wipe of all handrails, once a month, along all catwalk platforms located within the interstitials of building 1 interstitials. 6.1.5 Will remove all asbestos containing materials (ACM) and other debris from the catwalk systems and all rooms located within the building 1 Interstitials. 6.1.6 Report and take pictures of all violations where construction sites are left in an uncleanliness state. 6.2 Repair & Upkeep Damaged Encasement Within 3 Feet of Catwalk throughout all Interstitial floors. 6.2.1 Photo and document the encasement needing repaired with photos prior to and after completion and submit with invoice. 6.2.2 Report and take pictures of repaired damage to encasements and/or possible neglectful damage to encasements. 6.3 Maintain all clean rooms and entry points to clean rooms for serviceability. 6.3.1 Clean rooms shall be cleaned weekly and kept free of debris. 6.3.2 Clean rooms shall be maintained free from tears, cuts, rips, etc. and provide the separation from asbestos and clean areas as designed. 6.3.3 Report and take pictures of all violations where Clean Room and Entry Points are left in an uncleanliness state. 6.3.4 Report and take pictures of missing or damaged vacuum systems. 6.4 Maintain Cleaning Service of elevator P-3 by wiping down all surfaces two times a week with a damp rooms and entry points to clean rooms for serviceability. 6.4.1 Report and take pictures of all violations where elevators are left in an uncleanliness state. 6.4.2 Report and take pictures of missing or damaged vacuum systems. 6.5 On request, the contractor will purchase and procure parts, J-Box Covers/Plugs, Unistrut/Safety Caps, light Bulbs and other consumables. Once identified and authorized submit receipt for parts with invoice. 6.5.1 Report and take picture of all new violations in failing to replace J-Box covers/Plugs, or Unistrut/Safety Caps removals and not being replaced. 6.6 After initial repairs, the contractor will maintain penetration repairs on fire and Smoke rates walls and barriers located within the interstitials. 6.6.1 Report and take pictures of all violations where other contractors are leaving repairs, in a state that is not in compliance with the manufacture (HILTI) specification and recommendation. 6.6.2 Report and take pictures of new fire/smoke barrier/wall penetrations that contractors are not bringing to compliance by the end of the day. 6.7 Provide one person, 40 hour a week, during working hours, to act as a liaison between the contractor and the COR. This person will be responsible for: 6.7.1 Daily emptying of all trash/ACM suits at each clean room entry point. 6.7.1.1 Work as the Liaison between interstitial workers, the contractor, and COR; for contractor to better understand the required deficiencies. 6.7.1.2 Collect from workers the found discrepancies, disseminate priority of work to workers, develop supplies lists for both the contractor and the COR. 6.7.1.3 Assist with the inspections of clean rooms. 6.7.1.4 Maintain logbooks of all Items found in and disposed of throughout the interstitial s cleanup, penetrations / repairs needed and conducted, and encasement / encapsulations needing and conducted. 6.7.1.5 Assist in cleanup and/or removal of graffiti. 6.7.1.6 As directed to respond to other emergency and or other situations pertaining to the Scope of Work. 6.7.2 Report and take pictures of all violations where contractors left site in an uncleanliness state. 6.7.3 Report and take pictures of missing or damaged systems, items, and/or procedures. 6.7.4 Certified in 40-hour course in Asbestos Contractor Supervisor and Hilti Fire wall applications, to act as my advisor on all requirements / aspects pertaining to this contract. 6.8 Each year the contractor will supply the VA with two new easily mobile and compactible containment systems designed for one person to easily transport up and down the stairwells. This device will be used for inspections and emergency responses to interstitial wall door access. it will be self-contained and have the ability to provide a negative pressure. 6.9 On request, the contractor will purchase and procure Entry Hepa Vacuum parts, J-BBox Covers/Plugs, Unistrut/Safety Caps, and other consumables. 6.9.1 No parts or procurements can be authorized/committed to purchase or procure without the written authorization of the Contracting Officer Repersentive. 6.9.2 All invoices for this line Item will be submitted, to Tungsten network Invoice Payment Processing System, with item description and cost clearly identified on the invoice. 6.10 Upon completion, as directed you will be required to provide all manner, means, methods, equipment, expertise, incidentals, labor, materials, parts, services, supervision, tools, transportation, and qualified personnel necessary and required to perform the following in accordance/compliance with current NFPA and building codes, VA directives, OHASA regulations, industry guidelines / standards, and this statement of work (SOW) throughout the VA San Diego Healthcare System campus; personnel to conduct off catwalk cleanup of the interstitials. 6.11 Install an adequate Hepa Vacuums, on each floor, at each P-3 Clean Room entrance. 6.11.1 Installation will include ensuring power is supplied to the vacuums. 6.12 Build a clean room at each entry into the interstitials where Asbestos is located. Prior to build, plans will need to be certified by EH&S. 6.12.1 Clean room will be made of fire-retardant chloroplast sheets material and steel post construction with the capability to expand if necessary to bring in larger materials. 6.12.2 Each clean room design will be vetted through the Environment Health and Safety (EH&S) shop prior to starting construction. 6.12.3 At the conclusion of each clean room build, the contractor will have clean rooms certified through EH&S. 6.13 Repair & Upkeep all fire/smoke wall penetrations within the Interstitials. 6.13.1 Photo and document the repairs with photos prior to and after completion. 6.13.2 All off catwalk repairs are required to be coordinated with service below, prior to leaving the catwalk. 6.14 Removal of all ACM and Debris from the catwalk and all rooms located within the Interstitials. 6.14.1 All ACM and debris will be properly disposed of through the EH&S GEMS. 6.14.2 All ACM and debris will be documented through a logbook with identifiable description, EE# and Serial# if applicable. CONTRACTOR QUALIFICATIONS & OTHER RESPONSIBLITES: Contractor shall obtain all necessary licenses required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. Furthermore, contractor and contractor's employees performing on this contract shall have, but not limited to the following qualifications, experience, licenses, and certifications: 7.2.1 California Contractor License issued by the State of California in accordance with the provisions of Chapter 9 of Division 3 of the Business and Professions Code, as amended. 7.2.2 Contractor its employee(s) performing on this contract shall be asbestos certified because V ASDHCS has asbestos in the facility. This asbestos requirement only applies to personnel who will perform physical work in asbestos identified or laden areas. All other personnel that will not disturb or contact asbestos will not be required to have an asbestos certification. Contractor shall provide the following certificates for assigned personnel prior to the performance of the contract: 7.2.2.1 Current 40-hour Asbestos Supervisor Certification - for supervisors. 7.2.2.2 Current 16-hour Operation & Maintenance Asbestos Training- for workers. 7 .2.2.3 Current Respirator fit test. 7.3 The Government reserves the right to accept or reject Contractor s employee for the rendering of services. Complaints concerning Contract Personnel s performance or conduct will be dealt with by the Contractor and COR with the final decision being made by the Contracting Officer. SAFETY REQUIREMENTS: All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VASDHS shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137 Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. The VAMC San Diego building 1, main building, contains asbestos. Any worker entering these asbestos containing areas must have an asbestos certification of no less than 16 hours and a valid fit test certification. Certifications must be presented to the VA project manager before any work in an asbestos area may begin. CHANGES: The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. VA POLICY: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. PARKING POLICY: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. IDENTIFICATION BADGES: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. SMOKING POLICY: Smoking is not permitted within the VA Healthcare System facilities campus. NORMAL WORKING HOURS: Normal hours of operation are Monday through Friday from 7:30 am to 4:00 pm Pacific time. All work is expected to be performed generally during normal hours of coverage unless requested or approved by the COR or his/her designee. Before commencement of work, the Contractor shall confer with the VA COR and Contracting Office to agree on a sequence of procedures; means of access to premises and building. All work performed, to include delivery of materials and equipment, shall be made with minimal interference to Government operations and personnel. Work performed outside the normal hours of coverage must be approved by the COR or his/her designee. OVERTIME AND NATIONAL HOLIDAYS: Any overtime or holiday pay that may be entitled to the Contractor s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government.
