This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested in response to this notice and a written solicitation will NOT be issued. This solicitation is being issued as a Request for Quote (RFQ) # IHS1514659-CSS. Submit only written quotes for this RFQ. This solicitation is 100% ISBEE BUY INDIAN SET ASIDE. Evaluation: FAR 52.212-2 Evaluation -- Commercial Items (completed as follows):
(a) The Government will award a firm fixed price Blanket Purchase Agreement (BPA) resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, with the technical requirements and past history performance being more importance than pricing. The following factors shall be used to evaluate offers:

Pricing
- Pricing will be evaluated by multiplying the total estimated pounds to be disposed by the price per pound and multiplying the price per container by the price and adding the numbers together for a comprehensive price.
Technical
- Vendor shall have the ability to provide services in Oklahoma City, OK and Gallup, NM locations.
- Vendor shall have the personnel, materials, supplies, and equipment required to perform the work under the contract.
- Vendor shall provide per pound rate for hazardous and non-hazardous waste materials.
- Vendor shall have the ability to provide proof of general liability insurance
- Vendor shall provide D.O.T. Manifest training for EBS staff and Manifest records on an online site.
Past History Performance
- Vendor shall submit at minimum 3 past history performance records where similar services were provided.
VENDORS SHALL SUBMIT THE FOLLOWING:
- COMPLETED PRICE SCHEDULE (TO INCLUDE OUT YEARS)
- Combined Synopsis/Solicitation MEDICAL WASTE DISPOSAL FOR NSSC LOCATIONS
- INDIAN ECONOMIC ENTERPRISE FORM
- PROVIDE PROOF OF LICENSED AND CERTIFIED SERVICE FOR THE SAFE REMOVAL OF REGULATED MEDICAL WASTE (RMW), HAZARDOUS WASTE, AND UNIVERSAL WASTE DISPOSAL
- PROVIDE PROOF OF GENERAL LIABILITY INSURANCE
- PROVIDE PROOF OF MANIFEST TRAINING AND MANIFEST RECORDS FROM ONLINE SITE
- VENDORS SUBMITTING LIKE OR EQUAL ITEMS MUST SUBMIT DESCRIPTIVE LITERATURE SHOWING HOW THEIR PRODUCT MEETS OR EXCEEDS THE REQUIREMENTS BEING SOLICITED.
This solicitation will result in a firm fixed price Blanket Purchase Agreement pursuant to the terms and conditions below. Terms and conditions other than those stated will not be accepted. The above pricing is all inclusive.
