The North Dakota Department of Health, Division of Injury Prevention and Control is requesting proposals to implement the Senior Fall Prevention Project Stepping On program. There are three awards available. The available budget is $4,000 for each award. Each award requires the offeror to implement the Senior Fall Prevention Project Stepping On program at two sites within North Dakota during the time period of March 1, 2012 through December 31, 2012. An offeror may apply for more than one award. Should an offeror apply for all three awards, the offeror would be required to implement the Senior Fall Prevention Project Stepping On program at six sites within North Dakota during the time period of March 1, 2012 through December 31, 2012 at an available budget of $12,000.
