Specifications include, but are not limited to: The vendor will implement, provide and monitor evaluation services of the Department’s chronic disease programs (including diabetes, hypertension, obesity prevention); monitor chronic disease outcome indicators; conduct regular conference calls or web meetings to monitor evaluation activities; serve as a member of numerous Department coalitions and teams; provide program coordination; provide technical assistance; provide input into the annual chronic disease grants work plans and progress reports; develop documents to organize the annual work plan and progress report; prepare required documents and reports including the annual evaluation plan and evaluation plan report.
