Specifications include, but are not limited to: The Columbus Health Department is in need of commercial carpet cleaning and spot removal services at the following WIC Clinics: -79 Outerbelt St. Columbus, OH 43213. -3933 E. Livingston Columbus, OH 43227. -1204 West Broad Street Columbus, Ohio 43223. -3850 Sullivant Ave. Columbus, OH 43228. -3781 S. High St. Columbus, OH 43207. -1500 E. 17th Ave. Columbus, OH 43211. -4550 Indianola Ave Columbus, OH 43214. -5253 Cleveland Ave. Columbus, OH 43231. -1180 E. Main St. Columbus, OH 43205. The areas that need to be cleaned are commercial carpets located in private offices and in some cases, waiting rooms, as well as play rugs located in the waiting rooms. Spot removal and deodorizing will also be required. There will be one cleaning required before September 30, 2009. It is anticipated that cleaning will be required twice during the period beginning October 1, 2009 and ending September 30, 2010. All sites will be open for assessment on Thursday, April 2, 2009 from the hours of 1:30 - 4:00 pm. All bidders are responsible for their own precise measurements and assessment of the area. Bidding instructions: -This contract will be bid and awarded as one lot. One lot is defined as cost to clean all locations listed. One lot also equals one cleaning. This contract will be awarded with the option to renew for the possibility of two cleanings during the period October 1, 2009 through September 30, 2010. If the pricing is different during the second period, please indicate the price per cleaning in the comments section. This contract will be awarded on an all or none basis. Invoices 1. Invoices must be sent to Cindy Vesco, WIC Program, 240 Parsons Avenue upon completion of the services. 2. Each invoice must include the address where the service was performed, as well as what services were performed, and the total cost. Evaluation Criteria 1. The cost and pricing structure of the Bidder's proposal. Pursuant to Code Section 329.06(c)(10), Finance and Management Director has determined that the responsible wage for purposes of contracts to provide custodial, landscaping, guard and security services, and cleaning and recycling services is currently $12.36 per hour. In determining the best bidder for the purposes of awarding a contract for any such service, it is required that consideration be given to bidder's payment of a responsible wage to employees who will be performing the work specified in the bid solicitation unless the bidder is exempt because it employs 25 or fewer full-time employees or the procurement does not exceed $20,000 in the fiscal year. Any staff entering the WIC locations must be bonded and 18 years of age or over. HIPAA The Contractor will perform all services in accordance with the privacy regulations [45 CFR ?? 164.502(e); 164.504(e)] issued pursuant to the Health Insurance Portability and Accountability Act [42 USC ?? 1320 -1320d-8] and the terms of the attached Columbus Health Department Privacy Agreement. Following the award, vendors will be required to complete an EBO contract compliance application, a delinquent personal property tax affidavit, and will be required to present proof of worker's compensation, and a certificate of liability insurance. The terms and conditions specified in this Contract constitute the entire contract governing the purchase of services by the City from the Contractor, and shall supersede any terms and conditions which may accompany Contractor's invoice/bid/estimate. The terms of this Contract shall prevail over any conflicting or deficient terms or conditions listed in any attachments from the Contractor.
