1. Up to five (5) Family Mentors‐May be certified Peer Support Specialist. Will have had experience as a former Child Protective Services (CPS) client with prior substance abuse history, treatment and a minimum of 2 years documented sobriety and experience as a parent advocate/family mentor in any system. Mentors will be supervised by the contractor to provide administrative and clinical oversight and their day‐to‐day activities will be instructed in collaboration with a Social Work Supervisor at HHS. 2. Assist HHS in facilitating the START model by a.) Provide Clinical supervision of Family Mentors b.) Attendance at Child and Family Team Meetings by case assigned mentor, case staffing meetings as mandated by state child welfare policy by case assigned mentor, Monthly START leadership Team meetings by director of the contractor’s agency, monthly START Stakeholder meetings by Family Mentors and Clinical Supervisor. c.) Maintaining relationships with multiple providers across mental health and substance abuse continuum to ensure clients receive necessary individualized services. Communication with multiple providers to ensure needed information is delivered to the START Teams regarding progress or barriers in service delivery for families being served by START. d.) Participating as necessary in consultation and training with START Program Consultants. e.) Provide court testimony regarding interaction with clients in cases that have been petitioned by juvenile court. f.) provide and coordinate weekly Drug Screens for clients who are receiving START services to meet START mandates. Report the results of the drug screens to the START team and client. g.) provide transportation according to Minimal Work Guidelines for the START model. 3. Maintain extensive communication process to ensure that both HHS and Contractor are delivering the highest level of services possible and meeting expectations in client interactions as well as written product and tools. 4. Be responsible for addressing personnel issues expediently when these issues rise to the level of impacting success of the program as identified by either party. 5. Ensure all work is documented effectively within 7 days of case activity ( in compliance with State and CPS Policy mandates) to reflect that Minimal Work Guidelines and START Timeline have been met and ensure accurate data is shared to track outputs and outcomes related to START clients.
