a) Upon request by the Department via phone or email for a monitor, Vendor shall provide within two (2) business days via UPS or FEDEX delivery of the monitor to any of the Correctional Institutions statewide listed in Attachment J: Correctional Institutions List. b) Vendor shall provide UPS or FEDEX pickup for return of the device at no additional cost. c) Vendor shall provide on-site sleep disorder testing system that is CMS and FDA approved. d) Vendor shall provide a testing system that is HIPAA compliant. The Vendor shall share information and use information systems in a manner that ensures compliance with all state and federal laws, including but not limited to the Health Insurance Portability and Accountability Act of 1996 (HIPAA). See Attachment I: Business Associate Addendum and return signed copy with the RFP response and will be executed at time of award. e) Vendor shall provide a testing system to be utilized on an individual basis statewide and must be workable in any NCDAC Correctional Institution.
