Comprehensive Medical History - All necessary forms will be provided Medical Examination - Comprehensive physical exam includes: head, eyes, ears, nose, throat, neck, heart, lungs, gastrointestinal, genitourinary, lymph nodes, musculoskeletal, skin, neurological and hernia check. Vitals- Height, Weight and Blood Pressure Lab Analysis - Chemistry Screen, CBC, Lipid Panel, Thyroid and Urinalysis. Details on page 4.
