Description of Work: Repair of approximately 102 sites where the concrete infrastructure has been damaged by infrastructure installation. 1. These are small sites - many will be 5' x 5' or 8' x 1O' and in some instances may be curb or curb & gutter only. 2. They are scattered throughout the City limits and also in the County when associated with public water line installations/repairs. 3. There are usually a minimum of three trips to each cut - 1) Meet with the inspector to agree on the work to be done, 2) performing the work and 3) open the site up. 4. No patches can be left open overnight. A site must be poured the same day that it is dugout. 5. The contractor shall bill the City for work accepted by the City twice monthly. Billing dates shall be the 15th and the 3Oth or the last work day preceding these dates. The bill shall be an itemized bill that shall include the work order number, date of work, address, dimensions of each cut and the cost of each cut, line item#/description/quantity and line item cost as well as the total cost of each cut as well as the grand total for the invoice. 6. It is the contractor's responsibility to secure parking and to notify residents when and how long their driveways will be closed. 7. Because some of the sites are considered to be higher priorities because of location or age of the cut the contractor will be given 10 work orders at a time to be completed. New work orders will not be assigned until the previous 10 are completed. Failure to complete the work in a timely manner will result in liquidated damages.
