Specifications include, but are not limited to: The State of North Carolina Department of Health and Human Services (“Department”) Division of Human Resources seeks a Vendor to provide services for a comprehensive drug screening program which will support the Department's Alcohol and Drug Free Workplace Policy. The Department desires to contract with a third party who shall assume total responsibility for screening, chain-of-custody processing, laboratory analysis, medical review, and report findings to the Department. This program shall enable management to better document and confirm employee use and offer more effective rehabilitation with the ultimate goal of reducing workplace incidences. a) The Vendor shall establish collection sites within 10 miles of each of the thirty-six (36) cities/towns/zip codes, listed in the Collection Site Locations table referenced in Section 5.2.s, throughout the State, as well as provide proper collection training to personnel at each location. Indicate the name of the collection site located in each town/city listed below with your proposal response. b) The Vendor must be able to provide out-of-state testing sites for applicants Monday through Friday at no additional charge to the Department. Vendor must describe in its proposal how identifying out-of-state sites will be handled. c) The analysis of samples must be conducted by a laboratory that is certified by the Substance Abuse and Mental Health Services Administration (SAMHSA). d) Collection sites as identified by an asterisk (*) below must be available on a twenty-four (24) hour basis for department use and must be able to provide both urine and breath testing. 1. A critical part of this proposal is a method to allow management to quickly and easily undertake for cause testing at any hour of any day at a known location. 2. Access shall be available for urine samples for drug testing and breath testing for alcohol. This for cause testing must be conducted within four (4) hours of a request. 3. The proposal must specify whether the 24-hour sites are different from the regular office hour sites in these 8 locations and how Vendor will verify and consult with the Department’s Program Administrator and applicant/employee when issues arise. 4. Vendor must provide the name(s) of the contact person(s) and contact information for each 24-hour site in its proposal. e) The Vendor must maintain an up-to-date list of collection sites in the State’s desired locations and notify the Department's Program Administrator whenever changes occur.
